COIVD-19成为继2002年SARS、2012年MERS之后,由冠状病毒导致的第三次大规模流行病[1-2]。
疫情爆发后,受限于对新型冠状病毒肺炎传播机制、传播能力认知的不足,政府、医疗和公共卫生机构面临严峻的防疫考验。疫情初期,国内外学者利用有限的公开数据集对疫情的流行病参数进行了估计。文献[3]对9位早期确诊病例的潜伏期分析结果显示,其平均潜伏期约为5.1天。文献[4]分析了181例确诊患者数据发现,97.5%的患者会在接触病毒11.5天之内出现症状,表明采取14天隔离期是合理的。文献[5]对早期武汉425名确诊病例的回顾性研究表明,疾病平均潜伏期约为5.2天,平均传染周期约为7.5天,基本再生数R0约为2.2。这些回顾性统计研究为构建动力学模型推演疫情发展趋势提供了参数设置依据。文献[6]通过SEIR仓室模型,基于国内早期公开数据估算基本再生数R0为2.8~3.3;如以国外同行预测的感染人数为基准,R0在3.2~3.9之间。文献[7]建模研究估计R0值为2.68,并估算截至1月25日,武汉有75 815人感染COVID-19。文献[8]考虑对易感人群和隔离人群的刻画,建立了带隔离机制的SEIR模型,估算出R0高达6.47。此外,改进的SEIR仓室模型还被应用于拐点预测[9]和疫情发展趋势预测[10-11]。综合来看,基于动力学模型的研究普遍认为COVID-19早期致病传播能力高于SARS,属于中高度传染性的传播疾病,为有关部门提供了早期预警与决策支持。由于动力学模型的假设十分严格,其结果显著依赖于固定参数的初始值,难以充分考虑各种防控措施的动态变化以及执行效果对疫情发展的影响,因此疫情早期基于传播动力学模型的预测结果与疫情实际发展情况有一定的偏差[12]。随着疫情发展,很多研究工作利用人口迁徙数据对疫情发展与防控进行了分析。文献[13]对疫情爆发前期武汉外流人口去向的地理分布进行了统计分析。文献[14]的研究结果表明,COVID-19在中国的早期传播与湖北人口的迁徙高度相关,但得益于隔离措施,其感染率呈指数级下降。文献[15]使用海量手机用户定位数据,根据人口流动情况准确地预测了疫情爆发的时间、地点和强度。文献[16]基于人口流动大数据,研究了武汉“封城”对疫情阻断的效果,结果表明武汉“封城”极大地减少了全国各省的输入和感染人数,加速了全国各地进入一级响应状态的进程,有效控制了疫情在中国的传播。大量研究也表明,遵循社会疏离政策对控制海外疫情传播同样有效[17-20]。
自疫情爆发以来,专家学者在COVID-19的流行病学特征分析[21]、疫情发展趋势预测以及人口流动对疫情传播的影响[22-24]等方向展开了大量研究工作,有效推动了疫情的防控工作。然而,大部分研究是基于确诊、疑似和死亡等病例数据展开,确诊病例等数据的滞后性,对提前判断疫情发展并及时制定相关防疫策略提出了挑战。事实上,作为高风险人群,密切接触者人数蕴含了未来可能感染人数的变化趋势。追踪、隔离每一位密切接触者并为其提供最大程度的医疗服务,是疫情初期最具成效的防控措施[25]。目前,有关密切接触者的研究还少见报道,仅有部分关于集聚性病例流行病学特征的回顾性研究[26-29]。因此,本文通过分析密切接触者的数据,在疫情早期推断疫情发展趋势,为制定和完善新型冠状病毒肺炎防控措施提供判断依据。

COVID-19确诊病例的密切接触者被感染的机率直观反映了病毒的传染性,可以被视为除基本再生数之外,评估病毒传播能力的重要指标。目前,全国有关这方面的数据较少,仅北京市卫健委在每日疫情报告中统计了由密切接触者转诊为确诊病例的人数信息。基于该数据,本文计算了北京市每日新增确诊病例中源于密切接触者转诊的比例为:

${C_{{\rm{new}}}}$ 是每日新增的确诊人数;${C_{{\rm{trans}}}}$ 是每日新增确诊人数中,由密切接触者转为确诊病例的人数。由于2020年2月22日至发现境外输入病例较长一段时间内北京市无新增确诊病例报告,因此选取分析的日期范围为2020年1月26日−2020年2月21日。在此期间,$R$ 的平均值$\bar R$ =0.62,说明这段时期在北京市每日的新增确诊病例中,超过半数源于密切接触者。为进一步分析这段时间内$R$ 的分布情况,设置0.5为阈值,并定义一个二进制变量$L$ 用于区分阈值上下的$R$ 值,如果$R \geqslant 0.5$ ,则将$L$ 置为1,否则$L = 0$ 。通过$L$ 进一步计算转诊比例高于0.5的天数占这段时间窗口的比例${R_L}$ :$${R_L} = \frac{{\displaystyle\sum\limits_{i = 1}^n L_i }}{{{\rm{length}}}}$$ (2) 式中,
${L_i}$ 表示时间段内第$i$ 天的$L$ 值;length表示时间段的区间长度。计算得到${R_L} = 18/27 = 0.67$ ,该时间段内${R_L}$ 的分布如图1所示。结果表明,在此期间,67%的时间里有50%以上的新增确诊病例是由密切接触者转诊而来。由于北京市每日的数据基数小,波动较大,考虑通过取滑动平均值消除波动的影响,将时间窗口设为前后3天,由此可绘制出取滑动平均值之后R的变化趋势,以及新增确诊人数与由密切接触者转为确诊人数的变化趋势如图2所示。
分析图2a可以发现,春节期间,人员流动、接触频繁,在滞后的一个潜伏期(7~14天)内,新增确诊病例由密切接触者转诊的比例呈快速增长的趋势,并于2020年2月12日达到峰值,之后连续3天呈下降趋势,而后又出现短暂上升。事实上,由于疫情早期传播速度较快,密切接触者人数不断攀升,加之医疗资源紧张,疫情上升阶段接受核酸检测的人数是少于实际感染者人数的。随着2月中上旬“应收尽收”举措的实施,检测能力及效率大幅提高,有足够的医疗资源进行更大比例的密切接触者的检测,因此这一时期转诊比例有所回升。
同时也注意到,在疫情早期,除了由密切接触者转诊,还有一定数量的直接确诊病例未被追踪到,而在2020年2月12日转诊比例达到最高点之后,每日新增确诊人数几乎全由密切接触者转诊而来,如图2b所示,表现为新增确诊人数曲线与密切接触者转为确诊人数曲线几乎重合。这说明北京市1月24日启动的重大突发公共卫生事件一级响应有效阻止了疫情的进一步传播和扩散。在此期间,居民的主动居家隔离减少人员流动,国民公共安全意识的提高也保证了相关举措的顺利实施。经过一个潜伏期后,绝大部分新增确诊病例都来源于之前感染者的密切接触者,最大限度地阻断了疫情的传播途径。
Inferring the Trend of COVID-19 Epidemic with Close Contacts Counting

摘要: 密切接触者人数由于数据基数大且受统计口径变化影响较小,更能客观地反映新冠肺炎疫情的发展趋势。该文分析发现,2020年1月底−2月中旬北京市密切接触者转诊人数在新增确诊中的占比由50%提高接近100%,证实了严密筛查密切接触者并采取隔离措施对阻断新的传播风险的有效性。进一步对全国数据进行分析发现,全国累计密切接触人数逐步稳定在感染者数量的8倍左右,而新增密切接触者的增长率与5~6天后的新增确诊病例的增长率变化趋势一致。因此,追踪新增密切接触者的变化趋势,能提前一周左右推断新增确诊病例的变化,有利于各地医疗卫生机构做好应急准备工作,提前调度并储备相应救治物资,有效提升疫情防控效率。Abstract: Close contacts with high-risk exposure to COVID-19 cases are more robust in statistics for inferring future development of COVID-19 epidemic. In Beijing, the proportion of close contact cases in newly confirmed cases had increased from about 50% at the end of January to nearly 100% in mid-February, indicating that contact tracing and quarantine measures are effective non-pharmaceutical interventions for containing the epidemic. In addition, we show at the national level that the cumulative number of close contacts was stabilized at about eight times as much as infected individuals, and the growth rate of daily close contacts was consistent with that of daily confirmed cases 5~6 days later. Consequently, tracking the daily change of close contacts is beneficial to predict the trend of the epidemic, based on which advanced medical supplies scheduling and effective epidemic prevention can be achieved.
